FUTSAL POINT SALU KAWASAKI : COURT RENTAL REQUEST FORM
NOTE:
- Your request will be processed within 24 hours. Upon receipt of your reservation form we will inform you via email whether your application has been successful or not.
- Please make sure your contact details and request are correct before submitting.
- Please pay the rental fee upon check-in at the facility. Payment must be made in cash. No other means of payment is accepted at any Futsal Point facility.
- Please note our cancellation policy:
* Cancellation 8 days or longer before the reserved date - no charge
* Cancellation 4 to 7 days before the reserved date - 50% of total
fee due
* Cancellation within 3 days of the reserved date - 100% of total fee
due
- Reservation request in English is accepted only by using this form.
